This informational text is prepared to provide information to real persons (“Data Subjects”) about the processes, methods and purposes of collecting, processing, keeping, safeguarding and disposing of personal data and the rights and methods to exercise those rights pursuant to article 10 of the Personal Data Protection Law No. 6698 (“Law”) where Logo acts as a data controller during the presentation of the survey to Users.

Personal Data
As per the provisions of the Law, any data pertaining to an identified or identifiable person is personal data. The Participant/Customer may have submitted, disclosed and recorded, regarding unlimited companies, the company trade name, name and surname, telephone number, e-mail address and his/her position in the LOGO company to make use of the corporate digitalization measurement service and survey provided by Logo.
For Which Purposes Do We Process Your Personal Data?
1.1. Lawful Processing
As per article 5 of the Law, we are entitled to automatically process, either in full or in part, your Personal Data submitted, disclosed or recorded by you for legal reasons stipulating that “the personal data pertaining to contractual parties should be processed and the said processing is mandatory for the data controller to fulfill his/her legal obligation, provided that it directly relates to the conclusion or performance of the contract.”
2.2. Processing by Consent
Where you give your explicit consent as per legislative provisions, we may process your collected personal data in accordance with the articles of the Law for the conduct of advertising, marketing etc. and for the promotion of existing or new Logo and/or Logo Group Company products and/or services and/or features as part of the commercial electronic message consent already obtained where and when compulsory.
What Personal Data Collection Methods Do You Use and for Which Legal Reasons Do You Collect Personal Data?
We may collect and process your Personal Data either by your completion of the Digital Maturity Level Test Survey or via cookies within the scope specified in the Cookie Policy. The Personal Data collected may be saved in software, cloud, central server and Website database.
What Rights Do You Have As a Data Subject?
If you intend to exercise your rights in article 11 of the Law, you may deliver your requests to our Company in writing or by the registered electronic mail (KEP), secure electronic signature, mobile signature or your electronic mail address saved in our Company's system which has already been reported to our Company by you with the application form completed.
For written applications, you may send your original signed form completed in Turkish to our company at “Gebze Organize Sanayi Bölgesi, Şahabettin Bilgisu Cad. No:609 Gebze/Kocaeli” via a notary public or deliver it to us at the same address in person or via an attorney representing you.

You have to attach your identification documents and documents supporting your request, if any, to the form, and the copy of the notarized power of attorney including special powers if you want to exercise this right via your attorney.
If the information in the application is missing or wrong, the request is not submitted in a clear and comprehensible way, no supporting documents are submitted or the submitted supporting documents are not proper or the copy of the power of attorney is not attached to the form for the applications made via an attorney, then we may have problems in satisfaction of our requests and delays may occur during the research process. Therefore, it is essential that you comply with the requirements hereunder to exercise your rights. Otherwise, our company shall not be responsible for any delay. Our company reserves legal rights in the face of incorrect, untruthful/unlawful, malicious applications.
